Transplant Program

We provide strict collaboration with the Transplant Center in treating and managing pre- and post-transplant patients. We understand symptoms and treatment throughout pre- and post-transplant courses and provide continual education to the patient and family.

Objectives

• Improve Patient outcomes and quality of life

• GCHD Home management

• Ensure post-transplant sucess by focusing on signs and symptoms of rejection, infection, and complications specific to the organ.

• Pre & Post transplant management

(Heart, Lung, Pancreas, Liver, Colon, Stem Cell, etc.)

• Early intervention and evaluation during post-transplant course by observing assessing patient's subtle changes.

• Diligent teaching on immunosuppressants
